POSITION SUMMARY

The Clinical Pharmacy Assistant is a member of the healthcare team and is essential to providing patients with safe and effective medication therapy. The Clinical Pharmacy Assistant performs vital functions to support the patient care efforts working closely with the clinical team. This position reports to the Clinical Manager of the site.

PRIMARY ACCOUNTABILITIES

Achieve Results

  • Ensure the safe, productive, and efficient operation of the office assigned through timely and attentive administrative and clinical support (as authorized or licensed).
  • Ensure all patients enjoy a positive experience, treated with the care and compassion expected.
  • Ensure all patient records and related documents are managed and maintained timely, accurately, and consistent with all HIPAA and related regulations and requirements.
  • Participate in Patient Centered Medical Home care team as directed by team leader.

Operational Excellence

  • Ensure all tasks provided and associated with patient care, patient administrative processes, and related duties comply with all regulatory and accreditation standards, as well as clinical policies and procedures.

Relationships

  • Develop favorable relationships with all patients. Instruct patients and their family members training and /or information related to medication administration.
  • Establish favorable working relationships with all staff members associated with center operations, including medical receptionists, nurses, physicians and related staff.

Stewardship and Professionalism

  • Uphold and consistently represent the values and mission of the organization at all times. Represent the organization in a highly professional manner at all times.
  • Ensure compliance and attention to all corporate policies and procedures.

PRIMARY TASKS AND DUTIES

  • Reviews patient medication
  • Process third party billing claims and assist with prior authorization
  • Review medication history and complete reconciliation
  • Ensure compliance with regulatory requirements
  • Accurately complete medication refills and orders
  • Process electronic medication request
  • Review and update site formularies if changes and annually.
  • Have a working knowledge of PCMH process and goals.
  • Participate in daily huddle with PCMH care team staff to determine patients who are at high risk and/or are in need of PCMH care management.
  • Assists with office duties as assigned.
  • Provides service in a manner that is appropriate for the patient’s age; demonstrates knowledge and skills necessary to meet the patient’s physical, psychosocial, educational, and safety needs.
  • All other duties as assigned.

ESSENTIAL FUNCTIONS

· Read, write, and maintain patient records and related administrative documentation

· Knowledge and ability to use requisite tools, systems, technology and equipment in the collection of patient data

· Utilize computers in the collection, management, and reporting of patient records

· Ability to work with a diverse, patient population, assisting with positive health outcomes for the individual, family and community.

POSITION REQUIRMENTS

Education

  • High school diploma or equivalent.
  • Successful completion of a Pharmacy Technician certification and current licensure.

Experience

  • Demonstrated success with clear thinking and ability to reorganize as needed.
  • Demonstrated success in working independently, prioritizing and problem solving.
  • Demonstrated success in organization abilities.
  • Demonstrated success in computer skills including ability to use computer for scheduling, word documents and reasonable keyboard skills.
  • Demonstrated success in customer service/patient services or working with the general public, preferably in a medical care facility.
  • Significant knowledge of medication procedures.
  • Familiarity with the Patient Centered Medical Home model and the implementation of PCMH in ambulatory care.

Job Type: Full-time

Pay: $16.00 - $22.00 per hour
