Position Details:
· Job Title: Behavioral Health Coordinator
· Location: TBD
· Work Schedule: Monday – Friday
Qualifications:
· Excellent organizational, critical thinking, communication, interpersonal, and multitasking skills
· Strong documentation and computer skills
· Knowledgeable in developing and implementing care plans
· Ability to quickly learn and adapt to new technology
· Ability to work effectively with diverse patients and staff while promoting a positive image for CHANGE, Inc.
· Highly self-motivated with the ability to work independently.
· Current, unrestricted CMA Certification, LPN or RN license in West Virginia and Ohio
Responsibilities:
· The role of the Behavioral Health Coordinator is to assist the Team by coordinating care to patients on the assigned provider(s) daily schedule; proactively managing and coordinating care for patients not on the schedule; and offer preventive care for all patients who are part of the assigned panel.
