The RN Case Manager provides comprehensive case management services to ensure safe, effective, and timely patient care across the continuum. This position focuses on the coordination of clinical and psychosocial needs, resource utilization, and discharge planning in collaboration with the interdisciplinary team. The RN Case Manager serves as a patient advocate and leader to facilitate high-quality outcomes and patient satisfaction.
Essential Functions
- Perform comprehensive assessments of psychosocial and medical needs for assigned patients.
- Develop individualized case management plans of care that address clinical, psychosocial, and discharge needs.
- Coordinate and document the plan of care in the medical record, ensuring effective communication across all clinical disciplines.
- Act as a leader within the interdisciplinary team to manage care through criteria-based processes related to care level, patient status, and resource utilization.
- Facilitate interdisciplinary team meetings to collaborate on, implement, and evaluate patient care plans.
- Ensure patient selection for case management is criteria-based and that all interventions, assessments, and outcomes are thoroughly documented.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure seamless transitions of care.
- Educate patients and families on the plan of care, discharge instructions, and available community resources.
- Maintain compliance with hospital policies, accreditation standards, and applicable state and federal regulations.
Experience:
- Minimum of 3 years of recent Case Manager experience in an acute care setting (within the past year).
- Will also consider candidates with 3 years of experience in any of the following units:
- Medical/Surgical (Med/Surg)
- Telemetry
- Neuro
- Intensive Care Unit (ICU)
- Progressive Care Unit (PCU)
- Emergency Department (ED)
- Home health or insurance case management experience is acceptable if the candidate has:
- A total of 3 years of acute care experience, and
- At least 1 year of acute care experience within the last 5 years
- Associate’s Degree in Nursing (ADN) or Nursing Diploma – Required
- Bachelor’s Degree in Nursing (BSN) – Preferred
- Current Florida RN license or compact state RN license – Required
- If holding a compact license, must obtain an active Florida RN license within 90 days of hire.
- Advanced Practice Registered Nurse (APRN) license is acceptable if current and in compliance with state requirements.
- Case Management Certification: CCM (Certified Case Manager) or ACM (Accredited Case Manager)
- Nursing Certification: RN-BC (Board Certified)
- Utilization Review Certification
