About the RoleWe are seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract opportunity in Missoula, Montana. The ideal candidate will provide skilled nursing care, coordinate patient services, and support positive clinical outcomes through comprehensive home health case management.
ResponsibilitiesPerform comprehensive patient assessments and develop individualized care plans.
Provide skilled nursing care, including wound care, medication management, catheter care, and other clinical interventions.
Coordinate care with physicians, therapists, home health aides, and interdisciplinary team members.
Educate patients and families on disease management, medications, treatment plans, and home care needs.
Maintain accurate clinical documentation in Homecare Homebase (HCHB) while ensuring compliance with home health standards.
Active Montana Registered Nurse (RN) License required.
Minimum 2 years of recent Home Health, Hospice, or Acute Care nursing experience.
Valid driver's license, reliable transportation, and appropriate auto insurance required.
Experience with wound care, venipuncture, catheterization, and parenteral therapy preferred.
Strong assessment, communication, case management, and documentation skills.
