Contract: 13 Weeks
Schedule: Day Shift | Monday–Friday (Weekend Rotation Approximately Every 4th Week)

About the RoleWe are seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment in Barre, Vermont. This role provides individualized skilled nursing care in the home setting while serving as a Case Manager for assigned patients. The position requires strong assessment skills, care coordination, and experience with OASIS documentation to support quality patient outcomes.
ResponsibilitiesProvide skilled nursing care and comprehensive patient assessments in the home health setting.
Develop, implement, and coordinate individualized plans of care in collaboration with physicians and the interdisciplinary team.
Serve as the primary Case Manager for assigned patients and coordinate ongoing care services.
Complete accurate OASIS documentation and maintain patient records using Homecare Homebase (HCHB).
Educate patients and caregivers on disease management, medications, treatment plans, home care needs.
Active Vermont Registered Nurse (RN) License required.
Minimum 1 year of Home Health nursing experience required.
Current BLS/BCLS certification required.
OASIS experience required; Homecare Homebase (HCHB) experience preferred.
COVID-19 vaccination required in accordance with facility requirements.
