Pay Breakdown
Weekly Gross: $ 2,000 To $ 2,200 /Week
Blended Hourly Rate: $ 45 to $ 53/Hour
Weekly Taxable Pay: $ 1246
Required Qualifications & Certifications
Vermont Physical Therapist (PT) License Required Upon Acceptance
Minimum 2 Years of Physical Therapy Experience Required
Home Health Physical Therapy Experience with Patient Assessment, Treatment Planning, and Care Coordination Required
Experience Completing OASIS Assessments and Home Health Clinical Documentation Required
Additional Requirements
37 Guaranteed Hours Per Week | Monday–Friday Day Schedule
Provide Skilled Physical Therapy, Patient/Caregiver Education, Functional Assessments, and Individualized Plans of Care
Coordinate Care With Physicians, Nurses, Rehabilitation Staff, Caregivers, and Other Clinical Team Members
McKesson EMR Used; No Holiday Coverage Required
