Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team. This unique role will incorporate direct patient care in the field as well as management duties in the office. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support they need for optimal health and well-being. You will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans, monitor patient progress, and provide education on health management.
Responsibilities
- Conduct comprehensive patient assessments to determine healthcare needs.
- Develop and implement personalized care plans in collaboration with the healthcare team.
- Monitor patient conditions and vital signs, documenting changes accurately.
- Provide education and support to patients and families regarding health management and care plans.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Utilize EMR systems for efficient documentation and communication of patient information.
- Supervise and manage nurses and home health aides when applicable.
- Participate in case management activities, including budgeting and resource allocation for patient care.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Qualifications
- Knowledge of assisted living, home health, hospice care, acute care, and home care practices.
- Proficient in nurse management principles and supervising experience.
- Strong understanding of medical terminology, anatomy, physiology, and HIPAA regulations.
- Experience with documentation review, medical records management, and patient monitoring.
- Effective leadership skills with the ability to manage efficiently.
- Ability to create detailed care plans tailored to individual patient needs.
- Familiarity with Medicare guidelines related to home health services.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Unencumbered License as a Registered Nurse in Oklahoma required

Pay: $70,000.00 - $85,000.00 per year
