Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice team. The ideal candidate will possess strong clinical skills and a commitment to providing high-quality patient care. As an LPN, you will work under the supervision of registered nurses and physicians, assisting in the delivery of patient care in their home. Your role will involve direct patient interaction, ensuring comfort, safety, and support for patients.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Perform procedures such as education, catheterization and wound care as needed.
- Educate patients and their families on health management and post-discharge care.
- Participate in triage and emergency response situations as required.
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
Qualifications
- Current Licensed Vocational Nurse (LVN) license in Oklahoma is required.
- Previous experience in Hospice is preferred.
- Familiarity with electronic medical records (EMR) systems
- Strong understanding of patient assessment techniques and basic life support principles.
- Previous experience working with individuals in memory care settings is advantageous.
- Excellent communication skills and the ability to work collaboratively within a healthcare team.

Pay: $28.00 - $35.00 per hour
