OVERVIEW:
CVHS is seeking a clinical/operations hybrid pharmacist to work at CVHS- Charlotte Pharmacy to assist the clinical and operations pharmacy managers in clinical and operations pharmacy tasks, in accordance with the laws of the Commonwealth of Virginia, regulations adopted by the Board of Pharmacy, and the Drug Control Act. The clinical tasks will include performing detailed medication reconciliations, comprehensive medication reviews, and in-person medication therapy management reviews for clinic and pharmacy patients to identify patient specific medication therapy problems and help manage drug therapies and interventions to achieve positive patient outcomes. There are also opportunities to work on other clinical pharmacy programs. The operations tasks will include overseeing daily pharmacy operations, staffing workflows, scheduling, and distributive functions while ensuring safe, effective, and compliant pharmaceutical care to ensure precise prescription accuracy.
The pharmacist applicant must work as a team player and have strong communication skills that enable them to work closely with providers, nurses and other clinic and pharmacy staff. Applicant needs to have solid clinical training and be well-versed in pharmaceutical therapy to contribute to patient care plans. Applicant will also need to be proficient in utilizing resources to research various drugs, including new medications, and guidelines to learn more about their features, uses and side effects. This research is required to make effective, informed, individualized decisions regarding patient medications to ensure the best possible outcomes for patients. Applicant needs to also have a strong work history in the retail pharmacy setting.

Job Description:ESSENTIAL JOB FUNCTIONS:
Assists in supervision of all pharmacy activities as specified by the Drug Control Act.
Provides professional pharmacy services as appropriate.
Performs necessary compounding, using appropriate supplies, equipment and ingredients.
Assists in development of appropriate clinical pharmacy goals and objectives.
Maintains records as specified by the Drug Control Act.
Maintains and uses up-to-date patient drug profiles.
Assists in safe-guarding against diversion of drugs.
Assists in maintaining an up-to-date formulary; uses the formulary to dispense the proper generic or brand-name drug as prescribed by CVHS physician staff.
Assists in selection and control of all pharmacy drug stocks and supplies.
Assists in the administration of pharmacy audit procedures.
Provides patient medication counseling and education through the proper use of labels, accessory labels and personal contact.
Assists in the maintenance of day-to-day pharmacy records as specified by the Pharmacy Director.
All CVHS Pharmacy staff will abide by the standards of accreditation in Mail Order Pharmacy and Specialty Pharmacy.
Attends work as scheduled.
May perform other incidental tasks, as needed.

Qualifications:
EDUCATION, TRAINING AND EXPERIENCE:
Graduation from an accredited Pharmacy School with a Bachelor of Science in Pharmacy- or Pharm D.
Licensed to practice pharmacy in the Commonwealth of Virginia.
License is free of restrictions to bill Medicare and Medicaid.
