What are the responsibilities and job description for the Support Coordinator position at Central State Community Services Inc?
Interviews, hires and is responsible for regularly directing the work of five or more full-time equivalent direct support staff. Oversees enrollment in and completion of all training and inservices required by DDS and the person’s Team.
Monitors the records and programs of the person receiving supports. Completes a monthly program review by the tenth of each month that documents the person’s progress and status.
Participates as an active member of the person’s Team, acting as a liaison to all professionals and interested parties.
Completes the required agency and DDS number of on-site monitoring visits, and completes required documentation of such visits.
Attends and participates in agency and DDS meetings/training classes.
Provides continuous on-call emergency services to persons receiving supports and staff.
Is knowledgeable of and adheres to DDS, contract and agency policy and procedures.
Acts as a Health Care Coordinator [and/or back-up]. Monitors routine and special medical needs and fulfills all HCC duties required by DDS policy.
Assures ongoing eligibility for all benefits and reports monthly wages to the appropriate agency.
Monitors payment of all bills and expenditures.
Keeps continual contact with DHS-DDS case managers, parents/guardians and other family member of the person receiving services.
Provides direct support services on an as needed basis to the individual by completing all duties listed in the DSP Job Description. These direct supports may include providing transportation to people receiving services to their place of employment, to therapies/medical appointments or to leisure or activities using public transportation, agency leased vehicles or the employee’s own vehicle.
Assists individuals to secure and attend volunteer as well as other community vocation activities in their community in order to prepare for competitive employment.
