Regional Community Health Specialist

This regional position will work closely with the Population Health team, clinic partners, and community members to create healthy communities that directly impact the social determinants of health (SDoH) and promote health equity through policy, system, environmental and social change. This includes, but is not limited to, close networking with providers, social workers, clinic mangers, local businesses, non-profits, and state agencies. This position will engage and work closely with the Community Health Needs Assessment (CHNA) team to create a local community health improvement plan to address identified community needs, as well as those discovered through SDoH data review. This position will lead community and clinic collaboration to ensure internal and external resources are available to meet the needs of patients. Implements and abides by Customer Service Standards. Supports and implements patient safety practices as appropriate. Supports and demonstrates Family Centered Care principles when interacting with patients and their families and with co-workers.

Schedule:

• Full-time | 80 hours every two weeks | Hybrid work
• Days | 8am – 5pm

Pay and Benefits:

• Starting pay begins at $56,693.51 per year; exact wage determined by years of related experience
• Salary range: $56,693.51 - $85,018.35 per year
  • Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate

Qualifications:

• Bachelor's Degree in Community Health, Public health, Health Sciences, Social Work or other related healthcare field required; master's degree preferred
• 3 years of experience working in public health or related field required
• Experience in successfully working with diverse groups of community members, medical professional, and business partners
• Assessment planning and implementation, problem solving skills, program development and evaluation
• Understanding of, and experience with, the Community Health Needs assessment preferred
• Must exhibit excellence in written and verbal communication and interpersonal skills, be proficient in computer, social media and website skills and be responsive to email and partner requests
• Demonstrated skills including leadership, problem solving, communication, organization and goal setting required
• A valid Minnesota driver’s license, a driving record in good standing, and the minimum amount of insurance liability coverage required
