Join our team as a Case Manager and make a meaningful impact supporting individuals living with severe mental illness and/or substance use disorders. You’ll build strong relationships with clients, help them reach their goals, and connect them with the services and resources they need to thrive in the community.

What You’ll Do

• Build relationships and develop individualized, client-centered treatment plans.
• Provide supportive counseling, psychosocial rehabilitation, and crisis intervention.
• Collaborate with an integrated treatment team to coordinate care.
• Connect clients with healthcare providers, community resources, housing, and other support services.
• Support medication management and monitor client needs.
• Advocate for clients and provide education to families and natural supports.
• Help clients develop skills that promote stability, independence, and success in the community.
• Complete assessments, treatment plans, case notes, and required documentation.

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Bachelor’s degree in behavioral science or related field 

Minimum Required Experience:  1000 hours of post-degree clinical experience with individuals with severe and persistent mental illness

Travel Type: Road Warrior

Required Valid Driver’s License: Valid Wisconsin driver’s license  

Required Auto Insurance: Vehicle liability insurance in accordance with Agency policy. 
Knowledge, Skills, & Abilities:
•    Knowledge of Milwaukee County TCM, CCS and/or CSP/ACT Case Management.     
•    Experience with individuals with dual diagnoses is preferred. 
•    Experience with motivational interviewing is preferred. 
•    Knowledge of using work related equipment and applications such as cell phone and texting, computers, copy machine, Microsoft Office, and Teams. 
•    Effectively communicates and develops good working relationships with co-workers and outside agencies.   
•    Must be familiar with Microsoft Word, Excel, Teams and how to use a cell phone, computer, and fax machine.
•    Reliability in reporting to work regularly and on time. 
•    Must possess good communication, listening, writing and organizational skills. 
•    Effectively manages one’s own time, priorities, and resources to achieve goals. 
•    Build and maintain client satisfaction with the services offered by the organization. 
•    Diligently attends to details and pursues quality in accomplishing tasks. 
•    Ability to perform well in a fast-paced work environment. 
•    An understanding of mental health, substance use, and co-occurring needs and the ability to clearly communicate these to others.
•    Ability to support in emergency situations.
•    The ability to maintain and promote positive working relationships and work openly and effectively with one’s direct supervisor, colleagues, and outside agencies.   
•    Knowledge of Milwaukee County TCM, CCS and/or CSP/ACT Case Management is preferred.  
•    Access to a personal vehicle.

Physical Requirements, Visual Acuity, and Work Conditions: 

Physical Requirements: On occasion they must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Work is performed largely in the community but may at times include an office environment or other various settings.  

Visual Acuity: The worker is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures; transcribing; viewing a computer terminal.

Working Conditions: This is a community-based position. Staff are expected to utilize their personal vehicles to transport clients and meet with clients in their homes. This position may work in a variety of indoor and outdoor work environments which may include various outdoor weather conditions, loud noises, fluctuating temperatures, and the occasional use of stairs.
