Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This professional is responsible for delegating and directing all discipline-specific therapy services for the assigned patients, ensuring the design of functional treatment programs to allow for effective return to community activities. The role also involves educating patients, families, students, coworkers, and self while performing various professional duties associated with patient care.

This position operates on a 5x8.0 schedule, with evaluations being 60 minutes and follow-ups every 30 minutes.
Assignment Duration

Responsibilities
- Complete comprehensive evaluations of assigned patients.
- Utilize clinical reasoning to develop appropriate treatment plans and goals.
- Document care clearly, concisely, accurately, and timely, including all necessary records and entries.
- Participate in student programs, providing a positive learning environment with appropriate supervision.
- Actively create an individualized professional development plan and pursue continuing education.
- Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
- Understand and follow the organization’s performance improvement methodology.
- Degree in Physical Therapy from an accredited school.
- 6 months to 1 year of outpatient clinical experience.
- Current CPR certification.
- Valid state licensure for practicing in the designated location.
- Previous assignments with the agency or Select Medical are preferred.
