What are the responsibilities and job description for the Case Manager position at Cayuga Counseling Services Inc?
Cayuga Counseling Services, Inc. is seeking a Part-Time Case Manager in Cayuga County to provide Care Management under the Medicaid 1115 wavier program. This is an in-office position, and the hours are Monday - Friday 10am - 2pm.
Essential Functions:
- Conduct outreach to referrals per program guidelines.
- Navigate eligible members to programs within the Network.
- Review lease and housing documentation, ensure documentation meets all requirements and conducts outreach to follow up on paperwork needs in order to assist eligible members.
- Meets with clients to gather documentation and review program requirements.
- Addresses member's care plan goals.
- Completes surveys with clients who have been served through the wavier program.
- Maintains strict Compliance with HIPPA and all federal, state, and agency confidentiality regulations, policies, and procedures.
- Document all services and outreach per program policies and procedures.
- Attend all required trainings.
Qualification candidates:
- Detailed oriented, highly organized
- Able to communicate expectations clearly
- Strong computer skills, including experience with Microsoft and electronic documentation.
- Knowledge of social services and community resources
- Strong documentation and organizational skills.
- The ability to work independently.
