Schedule:

Hours: Monday- Friday 8:00- 4:30 p.m.

Rotating Bi-Weekly evening shift (one day) Mon - Thurs. 1:15pm - 10:00pm

Rotating weekend shift every 6 weeks Sat-Sun 9:30am - 8:00pm

Summary

The Case Manager is responsible for managing an assigned caseload of patients within a residential substance use treatment setting, while also providing active support within the therapeutic milieu. This role involves coordinating care in collaboration with counseling staff and interdisciplinary team members to ensure continuity and quality of services. The Case Manager supports the delivery of comprehensive, patient-centered, wraparound care, contributing to the structure and effectiveness of daily program operations and activities.

Essential Duties And Responsibilities

  • Maintain an average caseload of seven patients, providing consistent day-to-day support while assisting patients in achieving individualized treatment goals and completing required treatment-related tasks.
  • Coordinate with counseling staff regarding daily care and patient progress, and support case management needs such as transportation, education and legal requirements, referrals, access to community resources, daily living assistance, and skill development.
  • Implement and facilitate activities that promote patient engagement and retention, facilitate group sessions as assigned, and actively involve patients in developing independent living skills including budgeting, meal planning, hygiene, nutrition, and household responsibilities.
  • Supervise and participate in recreational, vocational, and educational activities, provide transportation to appointments and community activities, and ensure patients receive prompt medical attention when needed, completing all required documentation and incident reports.
  • Complete accurate and timely documentation in the electronic health record in accordance with OASAS standards, agency policies, and 42 CFR Part 2/HIPAA confidentiality requirements.
  • Provide crisis intervention services, safeguard and provide oversight of patient medications, obtain breath and urine samples for AOD testing, and utilize the on-call system when necessary.
  • Maintain knowledge of the facility’s physical plant and collaborate with other staff to engage patients in property-related tasks.
  • Participate in treatment team meetings, bi-weekly supervision, QA reviews, program improvement initiatives, staff meetings, trainings, and agency activities, including fundraising events.
  • Uphold the Canon of Ethics, maintain professional boundaries, foster positive community relations, and perform additional duties as assigned to support the overall mission of the program.
  • Other duties as assigned.

Requirements

QUALIFICATIONS

EDUCATION And/or EXPERIENCE

Required: Possess a bachelor's degree in social work, human services, or other closely related field or possess a CASAC (Credentialed Alcoholism and Substance Abuse Counselor) certification or be CASAC eligible with relative clinical experience in the substance use and/or mental health population.

CERTIFICATES, LICENSES, REGISTRATIONS

Required: Must maintain valid driver’s license and acceptable driving record.

Salary Description

$18.50-$20.00/hr
