What are the responsibilities and job description for the Care Manager HRSN position at Cayuga Counseling Services Inc?
Cayuga Counseling Services is seeking a Care Manager to provide Care Management for eligible Medicaid members under the Medicaid 1115 wavier program.
The Care Manager is responsible for the following:
- Provide Care Management services that address the health-related social needs of eligible members under the Medicaid 1115 waiver which can include housing, nutrition and transportation needs.
- Assist eligible members with securing safe, affordable housing.
- Follow up with members to ensure they receive the services they were referred to.
- Refer eligible members to programs within the Network.
- Maintain strict compliance with HIPPA and all federal, state and agency confidentiality regulations, policies and procedures.
- Document all services and outreach per program policies and procedures.
- Attend various meetings to educate providers about the services available under the wavier.
- Attend all required trainings.
- Performs other duties as needed.
Knowledge, Skills, and Abilities:
- Experience with working with Medicaid or vulnerable populations preferred.
- Strong computer skills, including experience with Microsoft Office and electronic documentation systems.
- Knowledge of social services and community resources.
- Strong documentation and organizational skills
- The ability to work independently.
The Agency offers:
