As a Transitional Care Coordinator at Catalytic Health Partners, you can play a crucial role in helping patients transition from hospital to home with our comprehensive 30-day program.

What We'll Expect from You and What You'll be Doing:
- Spending your day in healthcare facilities, educating patients about our 30-day program, which helps to ensure they get the recovery care they need.
- Clearly explaining the benefits of our program and how it caters to their unique healthcare needs.
- Gaining patient trust and consent for the program, ensuring they feel cared for and understood.
- Arranging the patient's first meeting with a nurse within a tight 5-day window.
- Regularly updating medical records and coordinating with the healthcare team for seamless patient care.
- Taking on additional responsibilities as required for effective patient support.
- Making calls to members and reaching out
Requirements:
- Legally authorized to work in the US.
- High school diploma or equivalent. Additional medical or behavioral health training is a plus. Medical Assistant certification is ideal, but not required.
- One year or more of experience in healthcare or patient management is preferred (but not required).
- Strong skills in organization, time management, and adaptability to change.
- Excellent communication skills, both in explaining complex information and in actively listening to patient needs.
- Empathy and teamwork skills are highly valued.
