Position Overview
As an LPN or RN on this case, you will provide 1:1 nursing care to a middle-aged adult in their home. This is a great opportunity for a nurse who enjoys relationship-based care, consistency, and having the time to truly focus on their patient's needs.
Key Responsibilities
Provide individualized skilled nursing care according to the patient's plan of care
Administer medications and treatments as ordered
Monitor and document the patient's condition and any changes
Assist with medical equipment and other skilled nursing needs as appropriate
Communicate with the patient, family, and Castor clinical team
Complete accurate electronic documentation
Provide compassionate, respectful care while promoting the patient's comfort, safety, and independence

Qualifications
Current Illinois LPN or RN license
Current CPR certification
Strong communication and documentation skills
Dependable, compassionate, and patient-centered
Private duty, home health, or complex-care experience is a plus, but training can be provided based on the needs of the case
