Job Summary:
As the Social Service Designee, you will assist the Social Service Director in the planning, developing, organizing, implementing and directing of the social service programs of this facility to assure that the emotional and social needs of the residents are met.
You will assist in reviewing complaints made by the residents, assist residents with information concerning their rights, living wills, etc. The Social Services Designee will make routine visits to residents and maintain contact with resident’s family, involving them with non-medical progress reports. You will assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required. Complete other duties as assigned by administration.

Education and Experience:
- Associates degree in related field preferred
- Experience working in healthcare and/or long-term care facility required
- Excellent customer service and communication skills required
