Position Overview:
Caring Hands Palliative and Hospice Care, Inc. is seeking a compassionate and dedicated Hospice Social Worker (MSW) to join our team in Prescott, AZ. This is a Per Diem position with flexibility and competitive pay. The Social Worker provides emotional support, counseling, and resource navigation for patients and families facing end-of-life challenges, working closely with the interdisciplinary team to ensure holistic care.
Key Responsibilities:
- Provide emotional support and counseling to patients and families coping with terminal illness
- Assist families in understanding and navigating the hospice process
- Conduct psychosocial assessments and contribute to individualized care plans
- Identify and connect patients and families with community resources, financial assistance, and support programs
- Document visits, assessments, and interventions accurately and in a timely manner
- Collaborate with interdisciplinary team members (RN, LVN, CNA, Chaplain, volunteers) to provide holistic patient care
- Support patients and families with advance care planning and end-of-life decision-making
- Offer bereavement support and follow-up as appropriate
- Participate in quality improvement activities and ongoing professional development
- Perform other duties as assigned by the Clinical Supervisor
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited program
- Current Arizona state license (LMSW or LCSW) required
- Hospice, palliative care, or healthcare experience preferred
- Excellent communication, counseling, and organizational skills
- Ability to work sensitively with diverse populations and family systems
- Valid driver’s license with reliable, insured transportation
- Understanding of hospice philosophy and compassion for the needs of the terminally ill
