Position Overview:
Caring Hands Palliative and Hospice Care, Inc. is seeking a compassionate and dedicated Chaplain / Spiritual Counselor to join our hospice team in Prescott, AZ. This is a Per Diem position with flexibility and competitive pay. The Chaplain provides emotional and spiritual support to patients and families, respecting diverse faith traditions, cultural practices, and personal values. Working as a vital member of the interdisciplinary team, the Chaplain helps ensure patients and families experience comfort, dignity, and meaning during the end-of-life journey.
Key Responsibilities:
- Provide spiritual counseling and support to patients and their families, regardless of faith background
- Facilitate grief support and emotional care throughout the hospice journey
- Participate in patient assessments and contribute to individualized care plans
- Document spiritual care visits and interventions accurately and in a timely manner
- Collaborate with the interdisciplinary team (RN, LVN, CNA, MSW, volunteers) to provide holistic care
- Lead or coordinate spiritual practices, rituals, or services when requested by patients/families
- Offer bereavement support and follow-up to families after the loss of a loved one
- Participate in quality improvement activities and ongoing professional development
- Perform other duties as assigned by the Clinical Supervisor
Qualifications:
- Graduate of an accredited theological or pastoral counseling program
- Ordination or endorsement from a recognized faith community preferred
- Experience providing spiritual care in hospice, healthcare, or pastoral settings
- Strong interpersonal and communication skills
- Ability to work sensitively with individuals and families of diverse faiths, beliefs, and cultures
- Valid driver’s license with reliable, insured transportation
- Understanding of hospice philosophy and the needs of the terminally ill
