Pay: $40.00 – $50.00 per hour | $70.00 – $80.00 per regular visit | $80.00 – $90.00 per recert visit | $140.00 – $180.00 per Start of Care (SOC)
Position Overview:
Caring Hands Palliative and Hospice Care, Inc. is seeking compassionate and experienced Registered Nurses (RNs) to join our hospice team in Prescott, AZ. This is a Per Diem position with flexibility and competitive pay. The RN is responsible for managing patient care, conducting assessments, developing care plans, and collaborating with the interdisciplinary team to provide exceptional end-of-life care.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide routine, recertification, and SOC (Start of Care) nursing visits
- Document patient services accurately and in a timely manner
- Educate patients and caregivers regarding diagnosis, treatment, and self-care
- Collaborate with interdisciplinary team members (LVN, Hospice Aide, MSW, SC, OT, PT, Dietitian, Clinical Supervisor)
- Deliver pain and symptom management, evaluating patient response to medications and treatments
- Ensure continuity of care across all services provided
- Participate in quality improvement activities and ongoing professional development
- Perform other duties as assigned by the Clinical Supervisor
Qualifications:
- Graduate of an accredited school of nursing (RN program)
- Current, active RN license in Arizona
- Hospice, palliative, or home health experience preferred (new grads with hospice interest welcome)
- Current CPR certification
- Strong communication, organizational, and clinical assessment skills
- Valid driver’s license with reliable, insured transportation
- Understanding of hospice philosophy and the needs of the terminally ill
