- Conducting client assessments and developing care plans based on nursing standards
- Managing and coordinating home health services in accordance with policies and community standards
- Responding to client needs and inquiries, reviewing referrals, and initiating appropriate actions
- Supervising and supporting clients through professional nursing practices
- Performing duties independently with accountability and adherence to regulatory guidelines
- Current RN license in the state of Colorado
- Minimum of two years' nursing experience, preferably in long-term care, dementia care, or acute care; home care experience is a plus
- CPR certification
- Educational background from an accredited nursing program; Baccalaureate preferred
- Strong communication, critical thinking, and organizational skills
- Ability to work independently and as part of a team
