- Deliver skilled physical therapy services in patients' homes
- Create and implement individualized treatment plans
- Educate patients and caregivers on exercises, safety, and mobility
- Collaborate with healthcare professionals to optimize patient outcomes
- Accurately document care in compliance with regulatory standards
- At least 1 year of experience as a Physical Therapist
- Valid Physical Therapist licensure
- Preferred home health or home care experience
- Valid driver’s license and auto insurance
- Strong clinical, communication, and patient education skills
