- Conducts psychosocial assessments, develops and implements social work plans, and provides therapy for individuals, families, and groups to promote emotional and social well-being.
- Manages cases by coordinating resources, offering referrals, advocacy, and education, ensuring appropriate patient placements.
- Assists in program development, policy implementation, and quality improvement initiatives. Conducts research, documentation, and departmental participation.
- Supports staff through consultation, training, and participation in departmental activities.
- Master's degree in social work and valid social work license (Licensed Social Worker or Licensed Clinical Social Worker in Hawaii).
- Basic Life Support certification from American Heart Association.
- Experience working with medical diagnoses and community agencies preferred, with at least one year of social work experience recommended.
