We seek compassionate Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high-quality in-home and telehealth care to patients with multiple chronic conditions such as diabetes, hypertension, chronic kidney disease, heart failure, COPD, and depression.
This role involves working within an integrated multidisciplinary team to establish long-term patient relationships, provide continuity of care, and support disease management, preventive care, and medication education.
Qualifications include an active license, board certification (NP: ANCC/AANP or PA: NCCPA), current DEA certificate or eligibility, and a minimum of 2 years of patient care experience. Field-based work requires reliable transportation, a valid driver’s license, and auto insurance.
Key skills include critical thinking, independent decision-making, knowledge of federal/state regulations (HIPAA, OSHA), and experience with EHR systems. Prior experience in managed care, population health, or comprehensive wellness visits is preferred.
