We are seeking a FACT Licensed Practical Nurse (LPN) to provide community-based mental health services in a multidisciplinary team setting.

- Conducting psychiatric assessments and evaluating physical health needs
- Coordinating referrals to community healthcare providers
- Ensuring proper medication self-administration alongside psychiatrists or APRNs
- Serving as primary case manager for a caseload, promoting recovery, empowerment, and self-determination
- Providing treatment, rehabilitation, and support services based on the FACT model
- Maintaining accurate, confidential clinical records
- Offering mental health education and crisis intervention
- Graduation from an accredited Practical Nursing program and valid Florida LPN license
- Minimum six months of medical/surgical or psychiatric nursing experience
- Proficiency with electronic health records, Microsoft Office, and internet use
- Strong communication, decision-making, and interpersonal skills
- Ability to work in diverse settings and respond effectively to emergencies
