We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.

- Building trusted relationships with participants and developing individualized support plans
- Assisting with program enrollment, updates, and educating participants about resources
- Monitoring health, safety, and program outcomes
- Promoting participant choice, independence, and community inclusion
- Collaborating with community organizations, healthcare providers, and support teams
- Maintaining accurate case documentation and ensuring compliance standards
- Conducting in-person visits and supporting functional screens as needed
- Bachelor’s degree in Social Work, Human Services, Psychology, or related fields with at least one year of experience working with individuals with disabilities or older adults, or four years of relevant experience
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with digital tools and a valid driver’s license for travel
- Preferred experience in case management, care coordination, or disability services
- Bilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued
