We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.
This role involves case management, care coordination, social work, and community-based support to help participants maintain independence, achieve personal goals, and exercise self-direction. Key responsibilities include building trusted relationships, developing and updating individualized support plans, conducting regular contacts and visits, promoting participant choice, monitoring health and safety, educating clients about program requirements, collaborating with community organizations, maintaining accurate documentation, and supporting annual assessments.
Qualifications: A bachelor's degree in related fields with at least one year of experience working with individuals with disabilities or older adults, or four or more years of direct service experience. Experience in case management, care coordination, or disability services is preferred. Candidates should possess strong communication skills, cultural awareness, and proficiency with technology. Bilingual candidates, especially in Spanish, Hmong, Arabic, Russian, or ASL, are encouraged to apply.
