- Providing comprehensive primary care services such as physical exams, preventive care, and chronic disease management
- Assessing, diagnosing, and developing individualized care plans in collaboration with interdisciplinary team members
- Educating patients and families on disease prevention, nutrition, wellness, and community resources
- Practicing across multiple clinic locations with occasional evening, weekend, or holiday coverage
- Master's degree and active Massachusetts licensure as an NP or PA-C
- Ability to manage diverse patient populations across the lifespan
- Strong communication, organizational, and relationship-building skills
