Duties
- Observe and monitor patients regularly, noting changes in condition and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Assist residents with activities of daily living such as bathing, dressing, grooming, toileting, and mobility using tools like Hoyer lifts when necessary
- Administer medications following prescribed protocols while maintaining strict compliance with HIPAA regulations and medical documentation standards
- Support residents with specialized care needs including Alzheimer’s disease, dementia, memory care, hospice care, and catheter care
- Take vital signs such as blood pressure, pulse, temperature, and respiration to track health status effectively
- Assist with meal preparation and feeding to ensure proper nutrition and hydration for all residents
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry and general cleaning
Requirements
- Valid CNA certification or PCA Certification with current CPR certification required
- Prior experience working in nursing homes, assisted living facilities, or home health environments preferred
- Familiarity with medical terminology and documentation practices within EMR/EHR systems
- Experience caring for individuals with developmental disabilities or memory impairments such as Alzheimer’s disease is highly desirable
- Ability to work with compassion and patience while supporting residents through their daily routines
- Knowledge of infection control procedures and resident safety protocols including hospice care considerations
- Strong communication skills to collaborate effectively with healthcare teams and family members
