We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to join our dedicated healthcare team. Responsibilities
- Conduct thorough patient assessments to determine needs and develop individualized care plans.
- Coordinate and manage hospice services, including symptom management, medication administration, and comfort measures.
- Collaborate with interdisciplinary teams to ensure seamless continuity of care across inpatient, outpatient, and post-acute settings.
- Educate patients and families about disease progression, treatment options, and end-of-life planning.
- Monitor patient progress and adjust care plans accordingly to optimize quality of life.
- Document patient interactions, assessments, and care plans accurately in medical records while maintaining HIPAA compliance.
- Facilitate discharge planning and transition of care to appropriate providers or facilities.
- Participate in utilization management and diagnostic evaluation to ensure appropriate resource utilization.
- Provide guidance on infection control protocols and maintain high standards of patient safety.
- Engage in ongoing professional development to stay current with healthcare best practices.
Skills
- Extensive clinical experience in critical care environments such as ICU, Level I & II Trauma Centers, PICU, or Emergency Room settings.
- Strong knowledge of physiology, medical terminology, and patient assessment techniques.
- Experience with hospital medicine, managed care, discharge planning, and medical records management.
- Familiarity with outpatient clinics, post-acute care settings, inpatient units, and outpatient services.
- Proven ability to manage complex cases involving geriatrics, behavioral health issues, infectious diseases, or palliative needs.
- Excellent communication skills for effective patient service and interdisciplinary collaboration.
- Ability to handle sensitive situations with compassion while maintaining professionalism under HIPAA regulations.
- Competence in diagnostic evaluation, utilization management, intake procedures, vital signs monitoring, and case management processes.
- Prior experience in medical-surgical nursing or hospital-based acute care is highly desirable. Pay: $90,000.00 - $100,000.00 per year
