Hospice Registered Nurse Case Manager

Job Overview
We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to join our dedicated healthcare team. In this role, you will provide comprehensive patient care, coordinate hospice services, and ensure quality end-of-life support for patients and their families. The ideal candidate will have a strong background in various clinical settings, including critical care, emergency medicine, and post-acute care, with a focus on delivering patient-centered care in a hospice environment. Responsibilities

• Conduct thorough patient assessments to determine needs and develop individualized care plans.
• Coordinate and manage hospice services, including symptom management, medication administration, and comfort measures.
• Collaborate with interdisciplinary teams to ensure seamless continuity of care across inpatient, outpatient, and post-acute settings.
• Educate patients and families about disease progression, treatment options, and end-of-life planning.
• Monitor patient progress and adjust care plans accordingly to optimize quality of life.
• Document patient interactions, assessments, and care plans accurately in medical records while maintaining HIPAA compliance.
• Facilitate discharge planning and transition of care to appropriate providers or facilities.
• Participate in utilization management and diagnostic evaluation to ensure appropriate resource utilization.
• Provide guidance on infection control protocols and maintain high standards of patient safety.
• Engage in ongoing professional development to stay current with healthcare best practices.

Skills

• Extensive clinical experience in critical care environments such as ICU, Level I & II Trauma Centers, PICU, or Emergency Room settings.
• Strong knowledge of physiology, medical terminology, and patient assessment techniques.
• Experience with hospital medicine, managed care, discharge planning, and medical records management.
• Familiarity with outpatient clinics, post-acute care settings, inpatient units, and outpatient services.
• Proven ability to manage complex cases involving geriatrics, behavioral health issues, infectious diseases, or palliative needs.
• Excellent communication skills for effective patient service and interdisciplinary collaboration.
• Ability to handle sensitive situations with compassion while maintaining professionalism under HIPAA regulations.
• Competence in diagnostic evaluation, utilization management, intake procedures, vital signs monitoring, and case management processes.
• Prior experience in medical-surgical nursing or hospital-based acute care is highly desirable. This role is vital for providing compassionate hospice care that respects the dignity of patients at the end of life while supporting their families through challenging times. The successful candidate will be committed to delivering exceptional patient service within a collaborative healthcare environment.

Pay: $90,000.00 - $100,000.00 per year
