Overview
We are seeking a dedicated and experienced Hospice Clinical Supervisor to lead and oversee patient care services within our hospice organization. Duties
- Supervise and coordinate the delivery of hospice patient care in accordance with physician orders and agency policies.
- Provide clinical guidance and support to nursing staff, home health aides, and other interdisciplinary team members.
- Conduct regular assessments of patient needs and adjust care plans accordingly to optimize comfort and quality of life.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient medical records and documentation.
- Oversee accurate medical documentation, including progress notes, care plans, and other required records, utilizing appropriate medical terminology.
- Collaborate with physicians, social workers, chaplains, and other team members to develop comprehensive care strategies.
- Monitor staff performance, provide training opportunities, and promote ongoing professional development.
- Ensure adherence to safety protocols related to physiology knowledge, anatomy understanding, and infection control practices.
- Facilitate communication between patients’ families and healthcare providers to support emotional well-being and informed decision-making.
- Maintain knowledge of medical records management systems and ensure all documentation complies with regulatory standards.
Skills
- Proven experience in patient care within a hospice or palliative setting.
- Strong understanding of physiology, anatomy, and medical terminology relevant to end-of-life care.
- Nursing licensure or equivalent healthcare certification is highly preferred.
- Knowledge of HIPAA regulations and best practices for medical documentation confidentiality.
- Excellent organizational skills with attention to detail in managing medical records and documentation.
- Ability to lead multidisciplinary teams effectively while demonstrating compassion and professionalism.
