We are currently seeking a PRN to part-time Speech Language Pathologist serving in a skilled nursing facility setting. This dynamic role offers variety, professional growth, and the chance to make a real difference in patients’ lives across the continuum of care.
Position Highlights
The Physical Therapist provides skilled physical therapy services to residents in a Skilled Nursing Facility (SNF), helping them regain or maintain mobility, reduce pain, and improve physical function. The PT evaluates, develops, and implements individualized treatment plans for residents with a variety of medical and functional impairments, with the goal of enhancing safety, independence, and quality of life.
Key Responsibilities
Perform comprehensive evaluations of residents to assess strength, mobility, balance, coordination, and functional abilities.
Develop and implement individualized care plans and therapeutic interventions based on clinical findings and physician orders.
Provide direct physical therapy treatment including therapeutic exercises, gait training, balance activities, and use of physical modalities (e.g., heat, electrical stimulation).
Monitor patient progress and modify treatment plans as needed to achieve measurable outcomes.
Educate residents, families, and facility staff on mobility techniques, fall prevention, and home exercise programs.
Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, and social services to support comprehensive care planning.
Participate in resident care conferences, discharge planning, and documentation of therapy services in compliance with Medicare/Medicaid and facility policies.
Recommend and train in the use of durable medical equipment (DME) as appropriate.
Maintain accurate, timely documentation in the electronic medical record (EMR) system..
Qualifications
Doctor of Physical Therapy (DPT) or Master’s degree from an accredited Physical Therapy program.
Current state license to practice as a Physical Therapist.
Previous experience in a skilled nursing or geriatric setting preferred.
Strong clinical judgment, documentation, and interpersonal communication skills.
Ability to work collaboratively as part of an interdisciplinary team.

Compensation
- Competitive salary - $38.00 to $62.00/hour depending on experience.
