Transportation Coordinator

CareCore at Heritage is seeking a reliable Transportation Coordinator to join our team. This position is responsible for safely transporting residents to appointments while coordinating schedules with healthcare providers, nursing staff, and families. Candidates with STNA or Direct Support Assistant (DSA) experience are strongly preferred.

Responsibilities

• Coordinate resident transportation for medical appointments and approved outings.
• Safely transport residents using facility vehicles.
• Assist residents with entering and exiting vehicles as needed.
• Communicate appointment schedules with nursing staff, providers, families, and transportation vendors.
• Maintain transportation logs, mileage records, and required documentation.
• Complete routine vehicle inspections and report maintenance concerns.
• Keep facility vehicles clean, fueled, and ready for use.
• Provide courteous and professional service to residents and families.
• Assist with resident support duties as needed.
  
  
  

Qualifications

• STNA or Direct Support Assistant (DSA) experience preferred.
• Valid driver's license with a safe driving record.
• Ability to assist residents with mobility needs.
• Excellent communication and organizational skills.
• Dependable, compassionate, and professional.
• Healthcare or long-term care experience preferred.

• Full-Time
• Monday-Friday
