This position plays a vital role in supporting residents and families through admissions, discharge planning, care coordination, emotional support, and advocacy throughout their stay.
Responsibilities:
- Coordinate and oversee social services for residents in a long term care setting
- Assist with admissions, discharge planning, and transition of care
- Support residents and families with emotional, social, and financial needs
- Collaborate with interdisciplinary team members to ensure quality resident outcomes
- Maintain compliance with state and federal regulations
- Advocate for resident rights and overall well-being
- Participate in care plan meetings and family communication
Qualifications:
- Current LSW license required
- Previous long term care or skilled nursing experience preferred
