Registered Nurse - Full Time - Home Care

$90,000 - $130,000/year
The Royal Care is seeking a compassionate and skilled Registered Nurse to join our growing home care team. As a Registered Nurse, you'll provide direct patient care, manage care plans, and coordinate with physicians, caregivers, and community health providers to deliver exceptional service to patients in their homes. Key Responsibilities
Conduct initial and follow-up patient assessments, evaluating physical, emotional, cognitive, and functional status
Assess family support, home safety, environmental factors, and patient education needs
Develop and maintain individualized care plans based on patient needs and goals
Maintain ongoing communication with physicians, caregivers, and community service providers
Supervise and evaluate home care aides and attendants
Ensure proper use and maintenance of adaptive/medical equipment
Document all care, services, and communications accurately in the medical record
Review medication profiles and coordinate changes with physicians
Develop discharge care plans and ensure smooth transitions of care
Educate patients, families, and caregivers on disease management, safety, and equipment use
Conduct scheduled reassessments and participate in quality assurance monitoring
Qualifications
Current New York State Registered Nurse (RN) license (required)
Bachelor's degree in Nursing (preferred)
Experience in home care with knowledge of relevant rules and regulations
Strong communication skills, both written and verbal
Ability to work independently and under pressure
Proven interpersonal skills and a patient-focused approach
Bilingual English/Spanish preferred
