Diabetes Lifestyle Management Specialist is responsible for promoting self management to assist the member with diabetes, achieve individual life's behavioral and lifestyle goals, to reduce risk, and optimize outcomes. The individual in this position will assess members for wellness education and disease management; introduce available tools to members to encourage personal health management; assist or lead in program development/projects; educate members regarding wellness and diabetes management; facilitate the coordination of care; and mentor staff.

Job Requirements
- Current, unrestricted license in the state of operations in one of the following: Registered Nurse (RN), Licensed Professional Counselor (LPC), Registered Dietician (RD) or Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Exercise Physiologist.
- 3 years of clinical experience.
- 1 years’ experience in case management or condition management in a managed care/health insurance company
- Chronic Care Professional (CCP) certification or Certified Case Manager (CCM) OR Certified Diabetes Care and Education Specialist (CDCES)
- Patient education experience.
- Computer/system software experience including Web.
- Verbal and written communication skills; analytical skills; sound clinical judgment.
- Bilingual in English\Spanish or English\Polish.
- Location: Must be located anywhere in the state of Texas
- Schedule: Monday - Friday 8am - 5pm CST
