Job Summary

Key Responsibilities
- Provide support in a community setting, ensuring performance expectations are meet SWG expectations and standards.
- Deliver job coaching and assist with skill development to enhance independence and employability
- Offer advocacy and communication support to ensure individual needs are met
- Supervise persons served at all times, maintaining a safe and structured environment
- Ensure assigned work tasks are completed accurately, meeting performance requirements
- Monitor and support individual performance, providing guidance as needed to meet expectations
- Maintain accurate and timely documentation, including daily case notes, progress tracking, and goal updates
- A genuine desire to make a positive impact in the lives of others.
- Supportive personality with a strong work ethic.
- No previous experience required; we offer comprehensive training.
- Valid driver's license and current auto insurance.
- Ability to safely transport individuals and work in a Human Services setting with favorable background screens.
