Qualifications:
- Must have prior long term care experience
- Master's Degree Required
- Ability to multitask
Job Duties:
- Plans and directs a comprehensive social services program which provides support services for residents and families
- Participates in an interdisciplinary team approach to better serve the needs of the residents
- Identifies and meets the psychosocial needs of residents by providing therapeutic interventions
- Performs an initial evaluation/assessment of each resident’s psychosocial needs to be placed in the medical record
- Develops social service component of the residents’ plan of care identifying specific needs, goals and approaches
- Completes timely documentation of social service interventions, assessments and progress notes in the resident’s medical record per facility policy and state and federal guidelines
- Acts as a liaison/advocate between residents, families, outside agencies, and the Administrator to ensure that the resident’s rights are maintained
- Submit continued stay requests to LDH
