Duties
- Assist residents with activities of daily living (ADLs), including personal care, grooming, dressing, and mobility support using patient transfer methods such as Hoyer lifts or manual handling techniques.
- Monitor patients’ vital signs, observe changes in health status, and document findings accurately to ensure proper care plans are followed.
- Administer medication and assist with medication management following HIPAA regulations and healthcare protocols.
- Prepare nutritious meals aligned with dietary needs, assist with cooking, and ensure a clean and safe dining environment.
- Support residents with memory care needs such as Alzheimer’s or dementia by implementing specialized care strategies and maintaining a safe environment.
- Provide transportation assistance for appointments or community activities using personal transportation or agency vehicles.
- Maintain cleanliness of living spaces through regular cleaning duties and ensure adherence to health standards in home & community care settings.
Qualifications
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Knowledge of patient observation techniques, patient monitoring, and dementia or Alzheimer’s care is essential.
- Experience working with individuals with developmental disabilities or neurodevelopmental disorders is advantageous.
- Certification in CPR, First Aid, manual handling, and medication administration is required or obtainable upon hire.
- Strong communication skills for effective client interaction and team collaboration.
- Ability to lift heavy objects safely and perform patient transfers using appropriate methods such as Hoyer lifts or other assistive devices.
- Familiarity with HIPAA regulations to ensure confidentiality of patient information.
