Responsibilities
- Assist clients with daily activities such as bathing, dressing, grooming, and mobility to promote independence and dignity.
- Administer medication reminders and monitor health status, reporting any changes or concerns promptly.
- Provide companionship and emotional support to reduce feelings of loneliness and improve mental well-being.
- Support clients with light housekeeping tasks, meal preparation, and transportation to appointments or errands.
- Follow individualized care plans meticulously, ensuring all client needs are met with professionalism and compassion.
- Maintain accurate documentation of care provided, including observations, changes in condition, and completed tasks.
- Collaborate with family members, healthcare professionals, and case managers to ensure comprehensive care delivery.
Experience
- Prior experience in home health care or personal caregiving is highly preferred.
- Knowledge of medical-surgical procedures and post-acute care practices enhances service quality.
- Experience working with diverse populations including geriatrics and individuals with chronic conditions is advantageous.
- Familiarity with emergency medicine protocols or Level I Trauma Center procedures adds value to emergency response capabilities.
- Background in hospital medicine or inpatient settings provides a strong foundation for understanding complex health needs.
- Demonstrated ability to work independently in outpatient or clinic environments ensures adaptability across various care settings.
