Responsibilities
- Accept patient care assignments readily.
- Introduce self appropriately (stating name & title) and communicate effectively with patients, patient significant other, and health team members.
- Assist the Registered Professional Nurse to collect data for completing the admission process.
- Assist the Registered Professional Nurse with data collection for use in the development and/or revision in the plan of care of patients throughout admission.
- Monitor patients’ condition including fluid intake and output and compose patient charts.
- Administer prescribed medication and perform glucose tests as per order. Observe patient under treatment to identify progress, side effects of medications.
- Should be able to lift 50 pounds and use appropriate techniques when moving patient.
- Communicate to the Registered Professional Nurse, changes in patient condition and/or patient's responses to interventions identified through observation, examination and/or patient complaint/report.
- Perform all necessary care (within scope of practice) required of the patient population by age, stage of development and diagnosis.
- Provide care being ever considerate of patient’s rights including but not limited to the right to confidentiality, privacy, and cultural and religious practices, values, customs and age.
- Assess, reinforce, and document patient/patient significant other teaching.
- Document clear, concise, legible and accurate nursing notes and other pertinent data which reflects implementation of patient care needs and responses.
- Maintain infection control practices according to policy and procedure i.e. Universal precautions, isolation precautions, etc.
- Assist with patient transfer and/or discharge.
- Maintain an understanding of the unit's quality assessment, improvement and risk management activities.
- Participate in quality assessment and improvement, risk management and organizational performance improvement activities as assigned, i.e. data collection, implementation of corrective action plans, participate in fire and disaster drills, etc.
- Maintains a safe and secure environment for patients.
- Attends and participates at unit-based staff & quality assessment, improvement and risk management meetings.
- Completes all mandatory in-service requirements within Department's specified time frame.
- Attends required in-service education programs and demonstrates learning through clinical performance, and educational programs.
- Communicates ongoing learning needs to the Charge Nurse, and/or Educator including Nursing Administration.
Skills
- Proven experience as LPN or similar role.
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.
- Familiarity with hospital health, safety and sanitation standards and procedures.
- Understanding of confidentiality obligations and nursing best practices.
- Excellent knowledge of medical and hospital terminology.
- Good knowledge of MS Office and data entry
- Excellent communication and interpersonal skills.
- Compassionate and able to handle stress.
Required Qualifications
- Successful completion of LPN license program.
- Current valid NYS LPN license/registration
- Current BLS certificate
Preferred Qualifications
- Experience in a psychiatric setting desirable.
- Pyxis experience preferred
