We’re hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide personalized, 1:1 care across Suffolk County.
What You’ll Do
- Deliver direct patient care and treatments based on individualized care plans
- Administer medications and tailored interventions
- Promote wellness through education, counseling, and preventive care
- Collaborate with multidisciplinary teams to coordinate care
- Maintain accurate and timely nursing documentation
- Assist with mobility and physical support (must be able to lift up to 50 lbs)
- Stay active and engaged during extended periods of standing or movement
- Take on additional duties as needed to support patient well-being
- Active LPN or RN license issued by New York State
- Minimum 6 months of experience in a home care setting
- Current CPR/BLS certification
- Strong communication and organizational skills
- Empathy, reliability, and a commitment to high-quality care
