Full Job Description: Evaluate and treat home health patients under plan of care orders and supervision of a licensed physician to assist patients in reaching their goals in collaboration with the interdisciplinary home health team.
Qualifications:
- Current license/registration by the state of hire as a Registered Nurse
- Preferred one year experience in Nursing
- Prior Hospice Experience also preferred but not required
- Ability to complete Competency and Proficiency Skills Evaluation
- CPR Certification
- Excellent verbal and written communication
- Valid drivers license with a clean driving record and a clean, functioning vehicle
