Registered Nurse Home Health and Hospice

SIGNIFICANT SIGN ON BONUS for full-time RN. Aspen Home Health and Hospice is seeking a compassionate and enthusiastic RN to join our rapidly growing team to work in our Idaho Falls and Blackfoot service location. After 5 years of full-time employment, RN will receive a company paid vacation for RN and a companion. Aspen uses state-of-the-art clinical software to do all documentation. Candidate must be self-motivated, have excellent communication and computer skills. Candidate must enjoy the elderly and relate positively to our patients and their families.

Responsibilities:

• Provide in-home home health and hospice visits including wound care, IV therapy, nursing assessments, patient and family instruction, lab draws, catheter care, ostomy care, tube feedings, pain and symptom management, and end of life care
• Have a thorough understanding Hospice HOPE, OASIS and key OASIS elements
• Assess and admit appropriate patients to Aspen Healthcare
• Be responsible to establish a plan of care with the patient, patient’s MD and team members to ensure patient safety and desired outcomes
• Coordinate and communicate with all disciplines providing care, patient goals and objectives
• Be responsible for accurate and timely documentation
• Attend patient care meetings and discuss patient’s progress towards goals
• Supervise CNAs
• Participate in weekend and on-call rotation

Skills and Prerequisites:

• Must have an active Idaho RN, current CPR and reliable car
• Experience in Home Health and Hospice Preferred
• Experience with HCHB EMR a plus
• 2 years of RN or LPN Hospital experience
• Must have good References

Pay: $38.00 - $41.00 per hour
