Bridges Health is seeking a self-motivated and productive admissions data entry professional to add to our Intake team. The Intake Clerk works with Long Term Care facilities across the state of Oklahoma and their care teams to complete all processes related to a new resident admission.
Responsibilities And Purpose
- Directly responsible and accountable for the completion of the entire admissions process.
- Taking call, setting up transports, communicate with receiving facilities.
- Coordination and regular communication with the Hospital Liaison Team and other referral sources, ensuring a smooth patient discharge and admission experience.
- Candidates having previous customer service experience and administrative skills in a nursing home environment are preferred, but for the right candidate not required.
- Must possess solid organizational skills with attention to detail while handling multiple duties/tasks.
- Knowledge of Skilled Nursing and Long Term Care services is a plus.
- Strong verbal and written communication skills are a must for a successful candidate.
- Must be a team player with the ability to establish effective working relationships with all team members.
- Must be self-motivated, have strong accountability and seek to meet and exceed goals.
