JOB PURPOSE:
The Intake Coordinator provides assistance with patient’s pre-certification and inquiries from insurance companies. Provides revenue information and commentary, identifying admission trends. Processes referred patients, conducts financial verification of the potential patient, and supports the facilities with the admission of referred patients. Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual’s needs and rights.
ESSENTIAL FUNCTIONS INCLUDE BUT NOT LIMITED TO:
- Receives referrals from physicians and facilities and initiates the intake process.
- Performs eligibility/insurance verification. Conducts timely financial verification for each referral.
- Obtains authorizations.
- Coordinates with the Patient Access Manager, Revenue Cycle team and Clinical team to ensure required information is obtained for patient records.
- Accomplishes efficient bed occupancy by designing, establishing, and maintaining procedures and policies to assure bed occupancy and control bed availability and patient traffic.
- Assists in preventing admissions conflicts by conferring with physicians on admissions priorities and bed availability; identifying issues needing resolution; resolving disagreements/conflicts or referring issues to others in a position to decide.
- Assures accuracy of patient data and fiscal information by reviewing audits and other reports of patient data and fiscal information.
- Identifies pertinent admitting trends by maintaining various categories of utilization statistical data within internal reporting systems.
- Serves and protects the hospital community by ensuring adherence to professional standards, hospital policies and procedures, federal, state, and local requirements, and CMS standards.
- Receives and processes referred patient fact sheet.
- Confirms receipt of referral with CL, verifies Medicare Reimbursement Days and/or insurance.
- Notifies facility designee to ensure appropriate equipment for the referral and communicates time of admission along with room assignment.
- Prepares and distributes referral packets to facility designee.
- Sends over discharge orders to accepting hospital from referring facility and distributes to charge nurse appropriately.
- Completes the admission process in accordance with approved policies and procedures.
- Tracks and communicates referral activity, including daily log of admissions.
- Prepares end of day admission/discharge report and distributes as directed.
- Distributes insurance authorization to Case Managers.
- Demonstrates an ability to be flexible, organized, and function well in stressful situations.
- Maintains a good working relationship both within the department and with other areas.
- Provides excellent customer service to all referring facilities, residents and family members.
- Other duties as requested within scope of practice.

EDUCATION/QUALIFICATIONS:
- High School diploma or GED required. Associates degree preferred.
- Minimum of five (5) years of experience as Admissions Coordinator.
- Minimum of three (3) years of experience as Intake Coordinator.
- Minimum of two (2) years of management experience in healthcare setting.
