Responsibilities
- Provide strategic oversight of the Enhanced Care Management Program
- Direct and support Lead Care Managers in delivering high-quality care
- Utilize data to monitor team performance and accountability
- Collaborate with HR to interview and onboard new staff
- Enforce compliance with internal policies and procedures
- Facilitate quarterly team meetings and training sessions
- Maintain open communication with team members and address concerns
- 5-7 years of progressive experience in a healthcare setting
- Management and supervisory experience
- Experience in interviewing, hiring, and conducting performance reviews
- Bachelor's degree
- Strong proficiency in Microsoft Office Suite
- Ability to travel to various site locations for training and meetings
- Bilingual in English/Spanish is preferred
