Patient Care Coordinator Responsibilities
- Support approximately 50 patients at any given time
- Coordinate care, appointments, and transportation for patients
- Engage with patients monthly to assess their conditions and goals
- Document and follow up on patient assessments and care plans
- Collaborate with clinical support teams as needed
- Minimum of 1 year of experience in the healthcare industry
- Healthcare experience is required
- Reliable transportation and a valid California driver's license are required
- Proficiency in Microsoft Office Suite, including basic formulas in Excel
- A high school diploma or a GED is required
- Bilingual in English/Spanish or Arabic is preferred
- A bachelor's degree is preferred
